Cancellation Policy
If you need to change or cancel your appointment, please do so with 24 hours notice. Otherwise you will be charged the full fee for your appointment. (Exceptions are now made if you have onset of cold-like symptoms, cough, fever, or any Covid-19 symptoms, and as always if you are in labor, but please do give as much notice as you can.)
New Patient Forms
If this is your first visit, please visit our ACUSIMPLE PATIENT PORTAL and take some time to fill out forms. You will need to fill out a Contact form, Informed Consent, and the appropriate Health History Form. If you are unable to do this, you may print the appropriate forms below (scroll down) and fill them out before arriving. In any case, please do scroll down to read important information about your visit, including location, parking, entrance, and so on.
For Acupuncture Sessions:
Please wear loose, comfortable clothing and refrain from any strong fragrances. It is best to have eaten prior to an acupuncture treatment, without being overly full. If you are taking pain medications for an acute condition, it is recommended not to have taken them right before your treatment (if possible), so that we can better assess your true condition. Also, please refrain from vigorous physical exercise, drugs, alcohol and hot tubs for several hours before and after an acupuncture treatment, to best allow your body to rebalance. Finally, please do not brush your tongue for a day or two before an acupuncture treatment, as tongue coating and texture are key diagnostic tools to assess your health. After treatment, some people feel relaxed, spacey, or euphoric. If so, sitting quietly for a few minutes and drinking water or tea is a good idea.

Directions, Entrance & Parking
The clinic is located at 2818 San Pablo Avenue in Berkeley, two blocks north of Ashby Ave, between Grayson and Oregon Streets, a big grayish blue A-frame a couple of doors down from Vital Vittles Bakery. If using GPS, google maps, etc, please include “Berkeley” in the address. The entrance to our downstairs clinic is found by going down the driveway on the left side of the building; the door to the clinic is in the back of the building. Parking is also in this back area, in a small lot bordered by a red brick wall and a gray brick wall; you may park perpendicular to the red brick wall, leaving room for 4 other cars to fit back there, please! If you do not find parking in the back, you may turn around and look for parking nearby; usually there is some within a block or two.
Access from the AC Transit bus lines, 72, 72M and 802. Closest BART station is Ashby.
Fees
All treatments are offered on a sliding scale for maximum accessibility. Please pay as much as you can generously and sustainably afford, while still being able to come in as frequently and regularly as needed. These fees are for Shoshana’s practice only; for other Manzanita practitioners, please contact the practitioner individually for fees, schedule, etc. Thank you so much!
Private Acupuncture Sessions: Initial Consultation & Treatment is $150-180; Follow-up Sessions are $105-135; With Hypnotherapy, add $60.
Community Acupuncture Sessions: Initial Consultation & Treatment is $65-105; Follow-up Sessions are $40-80 sliding scale.
Holistic Pediatric Visits: Initial Consultation & Treatment is $150-180; Follow-up Sessions are $100-130 (private session). Community Rate (shorter sessions, in community room, for follow-ups when appropriate and agreed upon) is $40-80 sliding scale.
Combination Acupuncture & Massage Sessions: Acu with half hour massage is $160-190*; Acu with 45 min massage is $185-215*; Acu with one hour massage is $210-240* … *Please ADD $45 if this is your initial acupuncture session.
Massage only: One hour massage is $125; limited appointments available.
